Please return the form below ONLY if you are unable to complete the home learning in any of the following ways:
• Accessed, completed and submitted on line.
• Printed and worked on then submitted via a photograph or scan of the work.
• Read from a screen, completed on a separate piece of paper and submitted via a
photograph or scan of the work.
NB The form will close at 3pm on Thursday 4 June. We will not be able to accommodate any requests after this date.
If you do request a pack it will be ready for collection from 1pm Monday 8th June.
